How Mitral Valve Changes Patients' Lives

Our patient with Cardiovascular Surgery shares his experiences about Mitral Valve treatment, Prof. Dr. Cengiz Köksal answers.

MITRAL VALVE

What is the Mitral Valve?

The mitral valve, located between the left atrium and left ventricle, controls blood flow from the lungs into the heart. It ensures blood moves from the left atrium to the left ventricle efficiently.

What Are Mitral Valve Diseases?

Degenerative valve disease results from structural issues like fibroelastic deficiency, Marfan syndrome, or Barlow's disease. It commonly leads to excessive valve movement.

Rheumatic Mitral Valve Disease

This disease stems from childhood rheumatic fever, damaging the heart valves. Symptoms typically appear after age 30, with both stenosis (narrowing) and regurgitation (leakage) possible.

Ischemic Mitral Valve Disease

Ischemic mitral valve regurgitation can develop due to narrowing of the coronary arteries or as a result of a previous heart attack.

How is Mitral Valve Disease Diagnosed?

Mitral valve repair preserves the patient’s own valve by correcting the malfunction. If repair isn’t possible, the valve is replaced with a prosthetic—either mechanical or biological.Mechanical valves last longer but require lifelong blood thinners. Biological valves need shorter-term blood thinners (3–6 months) and are better suited for patients over 65 or those who can’t take blood thinners or plan to become pregnant.Treatment choice depends on the patient's age, health, and lifestyle. If surgery isn’t immediately needed, regular monitoring is essential. Surgery is recommended in advanced cases.

What Are the Surgical Treatment Options for Mitral Valve Disease?

There are two main surgical options: mitral valve repair or mitral valve replacement. Globally, mitral valve repair is the preferred choice when feasible. If the disease is not advanced, medication may be used, but it does not provide a permanent solution.

What is the Difference Between Mitral Valve Repair and Replacement?

In mitral valve repair, the patient’s own valve is preserved and the malfunction is corrected, restoring normal valve function. If repair is not possible, a prosthetic valve is implanted to replace the mitral valve. There are two types of prosthetic valves: mechanical and biological. Patients with mechanical valves must take blood thinners for life. For biological valves, if there is no arrhythmia, blood thinners are only needed for 3-6 months. Biological valves may deteriorate over time, making them less suitable for younger patients but more appropriate for patients over 65.Biological valves may also be chosen for patients who may have issues with blood thinners or are planning pregnancy. Therefore, each patient should be assessed holistically to determine the most suitable treatment option. Patients who continue with medical management should be closely monitored. In advanced stages of mitral valve disease, surgery is recommended.

Cardiovascular Surgery

Endoscopic Aortic Valve Surgery

Endoscopic aortic valve surgery is a heart valve replacement procedure performed using minimally invasive techniques. Compared to traditional open-heart surgery, it is performed through smaller incisions, which speeds up the recovery process, reduces the risk of infection, and shortens the hospital stay. This method offers a comfortable and safe option for the treatment of aortic valve diseases.

Aortic Valve Repair and Replacement

Aortic valve repair and replacement are surgical procedures performed to resolve issues in the aortic valve, which regulates blood flow from the heart. The repair technique preserves the existing valve, whereas replacement involves using either a mechanical or biological valve. Both methods are selected based on the patient's condition and aim to improve quality of life.

Transcatheter Aortic Valve Implantation (TAVI)

TAVI is an innovative, non-surgical treatment method for replacing the aortic valve without the need for open-heart surgery. A prosthetic valve is inserted through the groin or chest, resulting in a shorter recovery time. It is an ideal option, particularly for elderly patients

Aortic valve regurgitation occurs when the heart's main valve fails to close properly, allowing blood to leak backward. Symptoms may include palpitations, shortness of breath, and fatigue. Treatment depends on the severity of the disease and may include medication, valve repair, or surgical valve replacement.

Aortic Valve Stenosis

Aortic valve stenosis is a serious heart condition that occurs when the aortic valve narrows, restricting blood flow from the heart to the body. Symptoms include shortness of breath, chest pain, and fatigue. Treatment options include medications, surgical valve repair, and minimally invasive procedures such as TAVI.

Peripheral Artery Disease

Peripheral artery disease is a serious circulatory disorder that occurs when blood flow decreases due to narrowing or blockage of the arteries in the legs. Symptoms typically include leg pain when walking, cramps, coldness, and non-healing wounds. Major risk factors include smoking, diabetes, high blood pressure, and high cholesterol. Treatment options include lifestyle changes, medications, angioplasty, and surgical interventions. Early diagnosis is critical to preventing serious complications and reducing the risk of limb loss.

Coronary Bypass Surgery

Coronary bypass surgery is performed to restore adequate blood flow to the heart by bypassing blocked or narrowed coronary arteries. Blood vessels taken from the chest, arm, or leg are used to create a new pathway. This method reduces the risk of heart attack, alleviates chest pain, and improves quality of life. With innovative techniques such as minimally invasive and robotic surgery, recovery times can be further shortened. It is an effective treatment option for patients diagnosed with coronary artery disease who do not respond to lifestyle changes.

Aesthetic Varicose Vein Treatment

Aesthetic varicose vein treatment offers modern solutions to eliminate varicose veins that cause discomfort and pain in the legs. Using minimally invasive methods such as laser, radiofrequency, foam sclerotherapy, and adhesives, varicose veins can be treated quickly and painlessly. Since these methods do not require surgery, recovery is fast, and patients can quickly return to daily activities. These treatments not only address health concerns but also improve the aesthetic appearance of the legs, leaving them smooth and healthy-looking.

Frequently Asked Questions

Cardiovascular Surgery for

What is mitral valve regurgitation?
Mitral valve regurgitation occurs when the valve between the left atrium and left ventricle does not close properly, causing blood to leak backward.
Do symptoms appear immediately?

Many patients may not experience symptoms for a long time.

What are the common symptoms?
Fatigue, reduced physical capacity, shortness of breath, and palpitations (arrhythmias).
How is it diagnosed?
An abnormal heart murmur heard during a physical exam may suggest the condition. Diagnosis is confirmed with an echocardiogram (ECHO), which evaluates heart function, valve structure, and leakage severity.
Is surgery always required?

No, a diagnosis of mitral valve regurgitation doesn’t always mean immediate surgery is needed. Treatment depends on the severity and symptoms.

When is surgery necessary for mitral valve disease?

Surgery is typically considered when symptoms worsen or become more noticeable.
